The Application form is in two parts:
- Part A : Details about yourself
- Part B : Details about your
prospective partner
To help you, MPI needs the information contained in both forms.
MPI IS YOUR BEST OPTION.
The forms can be filled in online in most browsers. However, if you
have an old browser or have any doubt, press Print BEFORE you fill in
the form just to make sure all fields are visible in the printout. If
the result is not satisfactory, download the WORD file and fill out the
form by hand.

To fill out the forms online, go to Part A and fill in the
information requested. DO NOT OVERRUN THE TEXT BOXES so that ALL your
input remains visible.
THEN PRINT OUT THE FORM BY PRESSING "PRINT" ON YOUR BROWSER.
 Repeat the process for Part B.
Nothing is sent to MPI when you complete the forms in your browser
and print them.
When you arrange your complimentary interview with Andrew, please come
with a copy of your form - both parts. Leave blank any questions
you are not comfortable with and where you have no strong preferences,
write "OPEN". A separate "Your Personality" form is completed
with Andrew at this meeting. If you live within 300km of Melbourne, bring
completed form with you to MPI. If you live over 300km from
Melbourne, you may post into MPI with 2 or more recent photos of you
for MPI's initial assessment. |
